Abstract. Infant and child skin is not simply miniature adult skin. At birth, the stratum corneum is thinner, corneocytes are smaller, natural moisturizing factor is lower, and the acidic surface mantle is not yet fully formed. Functional maturation occurs postnatally: structural differences largely resolve by age two, while the microbiome continues to develop toward an adult-like composition through approximately 18 months. The resident microbiota actively shapes barrier formation and lipid synthesis. In atopic dermatitis, microbial maturation is delayed, with persistently lower diversity even on non-lesional skin. These findings inform four core care principles: preserve vernix by delaying the first bath; cleanse gently with mild, pH-balanced, non-ionic formulations and microbiome-sparing preservatives; support barrier repair with emollients that boost ceramide levels and microbial richness; and manage the diaper area to prevent occlusion, maceration, and enzyme-induced irritation. Together, these evidence-based guidelines translate developmental dermatology into practical, daily care for infant skin health.

1. Introduction

The skin barrier of the newborn must convert, within minutes of birth, from an aqueous intra-uterine environment to a dry, microbe-rich, oxygen-rich external world []. The physiology of the barrier develops gradually during gestation, and full-term newborns are born with a well-defined SC, yet gradual functional maturation continues for months to years thereafter []. A clear, source-anchored picture of how infant/child skin differs from adult skin matters because topical products, diapers and wipes are applied to a surface whose permeability, pH, immunity and microbial ecology are still in flux . This review draws on five interrelated publications—a pharmacological/clinical update on infant barrier function , a review of epidermal immunity and its origins in utero , a review of the skin microbiota–physical-barrier relationship [3], a longitudinal 16S rRNA study of microbiome maturation in health and AD [4], and a randomized trial of emollient effects on the developing skin ecosystem [5]—to build a single, evidence-anchored narrative organized around two questions: (i) what are the measurable differences between infant/child and adult skin, (ii) what do those differences imply for care.

2. Structural and Compositional Differences

2.1 Anatomical maturation and layer dimensions

2.2 Vernix caseosa: a birth-coat with unique lipids

Figure 1. Ceramide profiles of vernix caseosa compared to stratum corneum (SC) at different developmental stages. (A) Comparison of vernix and adult SC (Rissmann et al., 2006). Ceramide AH was the most abundant in vernix, followed by NS, AS/NH, and EOS. Vernix showed higher relative levels of most ceramide species than adult SC, except for AP and NP, which were lower. Distribution patterns were similar between vernix and adult SC. (B) Comparison of vernix with fetal SC (16–20 weeks GA), mid-gestational SC (23–25 weeks GA), infant SC (1–11 months), and child SC (1–6 years) (Hoeger et al., 2002). In vernix, AH was the highest fraction, followed by AS, NS, EOS, and EOH, with AP and NP being the lowest. Vernix exhibited significantly higher AH and significantly lower NP and NS than infant SC (1–11 months) (p < 0.05). Vernix ceramide levels were comparable to those of premature infant SC (23–25 weeks GA), except for AP, which was significantly higher in vernix (p < 0.05).

2.3 Water, NMF and the acidic mantle

Infant skin holds a higher overall water content than adult skin, but hydration is lower at birth, peaks between 3 and 12 months of age, and water-holding capacity is lower than in adults . NMF is lower in infant skin than in adult skin at birth, yet by 2–3 months postpartum NMF, pyrrolidone carboxylic acid, histidine and proline are significantly higher in every infant group than in adults (p < 0.05) . This NMF rise is associated with a reduction in skin-surface pH, and epidermal acidification is described as necessary to promote colonization with an effective microbiota . Consistent with this, the adult-like "acid mantle" is not yet formed at birth, and newborn surface pH is higher (more alkaline/neutral) than in adults before it falls postnatally .

2.4 Pigment, photoprotection and sebaceous activity

Melanin is lower at birth and increases gradually through the first year; photoprotection is higher at birth but decreases drastically within the first few days; sebaceous activity is higher at birth and gradually decreases over the first few years . Epidermal Langerhans cells are not fully mature in early infancy .

2.5 Genomic signature of immaturity

Genome-wide comparison of newborn infant (n = 27) and adult (n = 43) full-thickness skin identified 1,086 differentially expressed probes (508 increased, 578 decreased in infants; |fold change| ≥ 1.5, adjusted p < 0.05) . Infant skin was enriched for extracellular-matrix (ECM) organization and development and for responses to lipid; adult skin was enriched for skin/epidermis development, keratinocyte differentiation, keratinization, cornification, and antigen processing/presentation . In other words, the infant barrier is still "in progress" and leans on innate/ECM defences, whereas the adult barrier is in a steady state of renewal and adaptive immune surveillance .

3. Functional Differences: Barrier Maturity, pH and Immunity

3.1 TEWL and the time-course of maturation

In full-term infants, barrier integrity (TEWL) is comparable to adults at birth, whereas in preterm infants TEWL is initially higher and decreases over time . By the second year of life, infant skin more closely resembles adult skin . Visual erythema is higher in both preterm and full-term infants than in adults at birth and remains higher at 2–3 months .

3.2 Innate versus adaptive immunity

Newborn skin is equipped to survive in a hostile post-natal environment largely through the innate immune system, including the ECM, while adaptive immunity matures later . Soon after birth, infants show higher expression of antimicrobial proteins (MPO in all infants; lactoferrin in late-preterm and full-term) and, by 2–3 months, significantly increased S100A7/A8/A9 and lysozyme versus adults . Conversely, adult skin is enriched for antigen processing and presentation and peptide-antigen binding . Thus the "immunological barrier" of the infant is dominated by innate, antimicrobial defences rather than the adaptive, antigen-presenting machinery that characterizes adult skin .

3.3 Why this matters for topical exposure

Because the infant SC is thinner, the vascular network is immature and (in preterm infants) TEWL is elevated, infant skin is more permeable to topically applied substances than adult skin . Products intended for infant skin should therefore minimize irritating or sensitizing ingredients and be free from microbial contamination .

4.The Skin Microbiome: Development and the Barrier–Microbiome Axis

4.1 The microbiota is an active barrier partner

The skin forms a complex, multi-level barrier comprising physical/mechanical, chemical, microbiological and immunological components [3]. Members of the skin microbiota are not mere occupants: they aid early neonatal adaptation and regulate keratinocyte differentiation (including expression of epidermal-differentiation-complex genes), SC formation and cell junctions [3]. Staphylococcus epidermidis expresses a sphingomyelinase that generates ceramides, directly feeding the barrier lipid pool [3]. Cutibacterium acnes has strain- and dose-dependent effects—low-dose exposure is barrier-stabilizing in vitro, whereas prolonged high-load coculture of certain strains is detrimental to keratinocytes and can lower claudin-1 expression, increasing paracellular permeability [3]. These findings reframe the microbiome as a guardian of the physical barrier rather than a bystander [3].

4.2 Healthy maturation: richness climbs toward adulthood

Figure 2. Relative abundance and amplicon sequence variant (ASV) richness of bacterial families stratified by age and skin site. Infants were aggregated by age and skin sites before being compared with adults (samples taken from the same clinic over the same time-period). (A) Relative abundance of bacterial families, showing that the composition remains relatively similar at the family level. (B) Richness increases in the healthy control infants towards adult levels after 18 months, but remained lower in the infants with atopic dermatitis (AD).

4.3 Atopic dermatitis: a delayed microbial maturation

Non-lesional AD skin showed significantly lower ASV richness (139.7 ASVs) than healthy control skin (187.7 ASVs; p = 0.007), with a significantly different community composition (p = 0.001) [4]. Unlike adult AD, the reduced diversity in infant AD was not driven by Staphylococcus aureus predominance; instead, the data suggest an altered, slower maturation of the AD skin microbiome toward an adult assemblage [4]. AD severity (O-SCORAD) correlated negatively with ASV richness (LRT 1.881, p = 0.017) when all AD sites were analysed together [4].

4.4 Emollient support of barrier and microbiome

Figure 3. Emollient support of barrier lipids and the developing microbiome. Panel A — Microbiome richness regression over the 5-week trial: richness rises in both regimens and is significantly higher in the wash-plus-lotion group by day 28 [5]. Panel B — Barrier-lipid response: the wash-plus-lotion regimen increases ceramides, free fatty acids and glycerol and reduces protein breakdown versus cleanser alone, while skin pH remains mildly acidic (4.5–6.0) [5].

5. Care Implications Derived from the Evidence

The differences above translate into a small set of concrete, source-linked care principles.

Defer the first bath to protect vernix. Because vernix is removed at the first wash and leaves skin drier and more vulnerable, the first bath should be delayed; WHO guidance cited in the barrier literature is not before six hours after birth .

Cleanse gently and keep pH near 5.5. Baby-wipe and cleanser formulations should use highly purified water, mild surfactants that effectively remove urine and feces without harsh agents—particularly avoiding sodium lauryl sulphate—and should maintain skin pH at or below ≈5.5 . Non-ionic surfactants (e.g., coco-glucoside, decyl glucoside, glyceryl stearate, polysorbate 20, sodium cocoamphoacetate) are among the mildest commonly used in wipes .

Choose preservatives that do not perturb the microbiome or sensitize. Preservatives should not alter the normal cutaneous microbiome and should be regulator-approved . In newborns, benzyl alcohol, benzoic acid and benzoates should be avoided because the metabolic pathway that handles them is not fully developed and skin exposure has been linked to increased jaundice risk . Formaldehyde-releasing preservatives (notably methylisothiazolinone and DMDM hydantoin) are associated with sensitization and allergic contact dermatitis in children and should be avoided .

Use emollients to support lipids and microbial richness. The randomized trial showed that adding an emollient to a gentle cleansing routine raised skin ceramides and microbial richness without disrupting the mildly acidic pH, and was well tolerated [5]. Emollients that replenish SC lipids and minimize friction are recommended in expert wipe-formulation guidance .

Figure 4. Skin permeability disruption of the diaper area. Occlusion of the SC (A) induced impaired skin barrier and increased skin permeability of penetrants (B), leading to an immunological response.

Treat preterm skin with extra caution. Preterm infants have higher initial TEWL and thinner SC, implying greater permeability, so formulations must be especially conservative [1,2].

Support, do not disrupt, microbial maturation. Because healthy microbiome richness climbs through the first ~18 months and AD is associated with delayed maturation and lower diversity, care routines that preserve microbial diversity—gentle cleansing, microbiome-neutral preservatives, emollient support—are preferable to aggressive antimicrobial or highly alkaline products [3–5].

6. Conclusion

Infant and child skin differs from adult skin along structural (thinner SC and epidermis, smaller corneocytes), compositional (lower NMF at birth, distinctive vernix lipids, higher water content but lower water-holding), functional (immature acid mantle, innate-biased immunity, time-dependent TEWL maturation) and microbial (age-dependent rise in diversity toward an adult assemblage by ~18 months) axes [1–4]. These differences are not defects but a programmed, post-natal maturation that is vulnerable to avoidable iatrogenic disruption [1,2]. The care agenda that follows is correspondingly specific: preserve vernix, cleanse mildly at pH ≈5.5 with microbiome-neutral preservatives, emollient to raise ceramides and diversity, and protect the diaper area from occlusion and enzymes [,5]. In AD, recognizing delayed microbial maturation opens the door to diversity-preserving care rather than merely antimicrobial approaches [4].
